The Community Health Center in Cowley County (CHCCC) provides comprehensive, patient‑centered medical services designed to support the health and well‑being of individuals and families throughout Cowley County.

Our medical services focus on primary and preventive care, serving as a medical home where patients can receive ongoing, coordinated care across all stages of life. Patients are encouraged to establish a long‑term relationship with a primary care provider who leads their care, supports informed decision‑making, and helps coordinate services both within and outside the health center. 

  • CHCCC offers routine and preventive medical services for patients of all ages, including: 

    • Annual wellness visits and physical exams 

    • Acute illness and injury care 

    • Chronic disease management (such as diabetes, hypertension, and asthma) 

    • Preventive screenings and health education 

    • Immunizations for all ages 

    • Women’s Health

    Care is provided with an emphasis on early intervention, prevention, and continuity, supporting long‑term health outcomes consistent with patient‑centered medical home principles. 

  • Medical services at CHCCC are delivered through a team‑based care model, where primary care providers work collaboratively with nursing staff, behavioral health professionals, care coordinators, and support staff. This integrated approach allows the care team to address physical, behavioral, and social factors that impact health, while promoting coordination and effective communication. 

  • When specialty services or additional care are needed, CHCCC assists patients through care coordination and referrals, helping reduce barriers related to access, navigation, and follow‑up. The health center works to ensure patients understand their care plans and receive appropriate services in a timely manner.

  • CHCCC is committed to reducing barriers to care. Medical services are offered regardless of insurance status, and eligible patients may qualify for reduced fees through the Sliding Fee Discount Program, based on family size and income. Additional enabling services, such as insurance enrollment assistance and interpretation, are available to support access and continuity of care. 

    Through comprehensive medical services grounded in prevention, coordination, and patient partnership, CHCCC serves as a trusted primary care provider and medical home for Cowley County residents.
